Clinical Observation of Peritonitis caused by Perforation of the Terminal Ileum due to Salmonellosis

Abstract
A clinical study. was done on 36 cases of peritonitis with, perforation of the ileum, out of, a total of 530 cases, of Salmonellosis, that were, admitted to the Inchon Christian Hospitalt during the 3 years period from January 1969 to December 1971, and the results are as followings:
L The incidence of perforation of the intestine in Salmonellosis was so high that 6.8% of cases showed peritonitis. out of a total of. 530 cases. (The remaining 494; cases (912,%) were patients without. perforation in Salmonellosis)
2. There was no decrease in the tendency of intestinal perforation due to Salmonellosis, among the 36"" cases, during the 3 years.
3. The occurance of Salmonellosis was most frequent in autumn.
4. Salmonellosis occured most frequently in those between 21 and 30 years of age and, the incidence in men was two times that in women.
5. The average period from the time of intestinal perforation to the time of operation was 1. 7 days and one half of the patients were admitted to Hospital within one day after perforation of the intestine.
6. In the clinical manifestations of most cases, varing intervals of fever were followed by abdominal pain, chillness and distention o¢æ the abdomen. General malaise, vomiting and bloody stool with diarrhea were also noted.
7. The mortality rate was high when the hematologic examination showed leukemia, neutropenia, lymphocytosis and an increased hemoglobin level.
8. Prior to operation, free air below the diaphragm in the simple abdominal X-ray appeared in 61. 1 % of perforated cases and the pattern of paralytic intestinal obstruction was seen 77. 8%. The incidence of complications was relatively high in childhood and old age and low in young adults and the middle aged.
9. The site. of intestinal perforation was within 60 cm. proximal to the ileocecal region in 97.1% of the perforated cases.
10. In surgical management, simple suture of the perforation site was performed in 86% of the cases and the other 14% of the 36 cases were operated on for resection of ileum including ileostomy.
II. Complications after surgery were infection, evisceration of wound (41.6%), intestinal fistula Cu.1%), complications of the lung (5.6% and obstruction of the bowel (2.8%). These complications were encountered more frequently in children and old men than in other age groups.
